Assistive Devices Program (ADP)
The Assistive Devices Program (ADP), supported by the Ontario government, helps people in the province with chronic issues and disabilities pay for customized equipment.
ProPT has a Registered ADP Authorizer
As a registered ADP authorizer, ProPT can assist with applications for the following devices:

Mobility Devices: Rollators/walkers for impaired mobility

Who Qualifies?
To be eligible for the program, you must:

Be an Ontario resident.

Have a valid Ontario health card.

Have a disability requiring the equipment or supplies for 6 months or longer.
Your income is not considered in the qualification process.Role of the ADP Authorizer (ProPT)
As your ADP authorizer, we will guide you through the process by:

Recommending ADP-approved assistive devices.

Completing the ADP application form with you.
-
Providing a list of approved ADP vendors.

Following up to ensure your complete satisfaction with the device.
